Working in close collaboration with Telehealth clinical leaders and champions, the Telehealth Program Coordinator will plan and implement Telehealth programs at Asian Health Services to facilitate access and utilization of digital health technologies to improve quality of care, patient outcomes, and population health. This position is responsible for planning and executing related project and program operations through effective coordination and collaboration with clinical and administrative leaders and staff as well as supporting teams from IS, HIT, and IT.
ESSENTIAL JOB FUNCTIONS:
Under the direction of the QI Director and Telehealth clinical leaders, coordinate and facilitate day-to-day activities of the Telehealth program, including:Manage schedules of Telehealth team members and their work across sites, including Chronic Care RN, Health Coaches, Digital Health Advocates, and others.
Assist in onboarding, orientation, and ongoing training of program staff, and volunteers, working in close collaboration with cross-site staff supervisors and managers.
Provide back-up for duties and responsibilities of Digital Health Advocates, if needed. This includes educating patients in access and use of Digital Health Technologies either in the clinic or individual patient’s home settings using in-person or virtual care visits.
Maintain expertise infrequently used technology for patient care, including video platforms, smartphones, and remote patient monitoring devices.
Facilitate program expansion to new patient cohorts and continue previous cohorts in the maintenance phase.
Support development of procedures and standards to maintain smooth operations and high quality of service. Monitor program quality measures to assure standards are met.
Support team in tracking and resolving technical issues.
Track and maintain inventory and supply for the Telehealth program, including remote monitoring devices, smartphones, and other technology, working with IT, IS, and HIT teams, as required.
Track program expenses to stay within budget.
Plan and assist in program promotion, and outreach to patients.
Oversee collection of operational data, including documentation of patient interactions in the Electronic Health Record.
Support telehealth leaders in project management, including coordinating project meetings, planning activities, and deadlines, collaborating with distributed clinical, IS, HIT, and IT teams, escalating issues and risks, as appropriate, and developing stakeholder communication tools. This includes the implementation of Plan-Do-Study-Act (PDSA) cycles to support continuous improvement.
Assist QI Director in using QI tools and approaches to evaluate and improve patient experience and program delivery, while supporting grant requests, and meeting reporting obligations for the awarded telehealth-related grants.
Scan external programs for best practices, gather data to inform operational and strategic decisions related to program development.
Assist in the design and development of new programs and services that are responsive to the patients and clients served.
Complete contract maintenance, data collection, and the collection of required billing documentation for funder/regulatory reports if indicated.
Participate in training programs, including telehealth
Demonstrate sensitivity and responsiveness to cultural differences.
Perform additional duties as assigned by the Telehealth Project Directors and Sponsor.
